A case of acute cerebral infarction in a patient with minimal change nephrotic syndrome

|
Nephrotic syndrome is associated with a hypercoagulable state and is a known risk factor for arterial and venous thrombosis. However, acute cerebral infarction secondary to arterial thrombosis in an adult with acute presentation of minimal change disease is rare. Here, we report the case of a 59-year-old man with acute proximal middle cerebral artery infarction within ten days of nephrotic syndrome. His clinical and pathological findings were consistent with minimal change nephrotic syndrome. The patient did not show any other predisposing factor to vascular thromboembolic events except for hypercoagulability related to nephrotic syndrome. The patient completely recovered from nephrotic syndrome and infarction upon standard treatment with prednisolone, diuretics, and antiplatelet drugs. The possible pathogenetic mechanism of this condition is discussed and the pertinent literature is reviewed. (Korean J Med 75:S881-S884, 2008)
